Aftercare and Transitional Support

The Aftercare and Transitional Support Program assists youth transitioning out of the care of the Children’s Aid Society. Our Aftercare and Transitional Support program strives to meet one’s physical, cultural, social, emotional, economic and spiritual needs within the context of relationships with family, friends, employers and community. Thus, through our work with the youth involved in the aftercare program, we continuously promote the view of interdependence and helping the youth develop a system of supports.

Youth workers provide support with housing assistance, informal counselling, college/university enrolment, independent living skill development, career planning, court appearances, issues related to substance abuse and job searching. Our work with youth is based on doing things with the youth rather than doing things for them. The youth will take the lead in defining the issues. The workers will provide face-to-face contact that will have a much greater impact on success.

Youth In Transition Worker

Similar to the Aftercare & Transitional Support Program, the Youth in Transition Worker program meets the needs of youth transitioning out of care through the York Region Children’s Aid Society, as well as community youth living in the York Region area between 16-24 years of age. The youth worker provides support with many domains of a young person’s life including; housing assistance, informal counselling, college/university enrolment, independent living skill development, career planning, court appearances, issues related to substance abuse and job searching. In addition, the YITW worker will connect youth to other supports in the community, based on client’s need for service.
